SOUTHERN OHIO SIGHTHOUND RESCUE ("S.O.S. Rescue") is a private adoption organization dedicated to placing neglected, abandoned, or unwanted dogs into caring, loving and permanent homes.

To adopt a dog through SOS, you must first complete a detailed application form that asks why you want a dog, how you plan to care for the dog and what kind of home you will provide. The required information is solely for SOS's use in determining your home's suitability to adopt any of our dogs; it will not be used by anyone outside our rescue without your consent. We screen the application for approval. If we determine that you cannot provide a suitable home for one of our rescues, you will be declined.

SOS Rescue will do their best to help you find a dog; however, we cannot guarantee a successful placement. You should expect to spend some time working with us to find the best dog for you. Please be aware that the more you narrow the criteria, the longer it may take to find a suitable dog.

SOS Rescue does ask for an adoption fee in order to help us cover expenses incurred.

All of the above information I have given is true and complete. Should an unneutered or unspayed dog be placed with me, I agree to have it neutered or spayed by such date agreed upon by me and SOS Rescue. At that time, I will forward evidence to SOS Rescue that such procedure has been completed.

This dog will reside in my home as a pet. I will provide it with adequate food, water, shelter, training, affection, and medical care. I understand that it is my responsibility to evaluate the dog for myself before agreeing to adopt it. I am in full agreement with these terms of adoption. SOS Rescue shall not be liable for, and is hereby relieved from, all liability for any damage, expenses, causes of action, fines, suits, demands, judgments or claims of any nature whatsoever arising from or by reason of any damage to property or injury to any persons caused in whole or in part by the dog placed in your home. I hereby accept and assume such liability and agree to protect, indemnify and hold ARPH harmless from and against all of the aforesaid. Submitting this form constitutes your permission for us to check references.
